Medicare is a federal program, but the most important consumer protections in Medigap are state law. Birthday rules, anniversary rules, guaranteed-issue triggers beyond the federal minimum, under-65 access, and rating-method constraints all vary state by state. The state reference cards below catalog those rules with primary-source citations.

Each state page also lists the State Health Insurance Assistance Program (SHIP) for that state. SHIP counselors are free, federally funded, and not affiliated with insurance sales. They are the right first call for one-on-one help.
